This form is for CMAQ Projects with work phases that are currently deferred, phases that sunset in 2012 and phases scheduled for federal fiscal year 2012 or 2013, regardless of sunset year (i.e. programmed in FFY 2012 or FFY 2013). Sponsors are required to submit status updates in May and October each year regarding whether those phases have been accomplished. If a status update is not completed the project will be considered for removal from the program.

Project sponsors are encouraged to use the custom hyperlink for each project/phase that was received from their PL or CMAP staff to complete the form.  Any questions about this form should be directed to your Planning Liaison.

Forms must be completed by October 15, 2012.  Projects/phases for which no status update is recieved by October 15, 2012 will be considered for deferral or removal from the CMAQ Program.
