This form is for CMAQ Projects with work phases scheduled for federal fiscal year 2012 (i.e. programmed in FFY 2012). Sponsors are required to submit status updates in May and October each year regarding whether those phases have been accomplished. If a status update is not completed the project will be considered for removal from the program.

Work phases scheduled in 2012are required to be accomplished (obligated) by the end of federal fiscal year 2012 (September 30, 2012). Additionally, if the phase was moved to FFY 2012 from a prior year an obligation must have occured to avoid removal from the program. Any questions about this form should be directed to your Planning Liaison.

Forms must be completed by May 15, 2012.  Projects/phases for which no status update is recieved by May 15, 2012 will be considered for removal from the CMAQ Program.
