CMAQ Project Status Update Form

This form is used to collect data regarding whether CMAQ Projects with scheduled phases in 2009 have accomplished those phases.  Phases in 2009 are scheduled to be completed by the end of Federal Fiscal Year 2009 (September 30, 2009).

Sponsors with a phase in 2009 have received a letter stating an update is required.  To see a list of all projects with phases in 2009, please click here.  This report shows not only phases scheduled in 2009 but also obligations on file for the project.  If the obligation matches the phase schedule in 2009, please still complete this form with the obligation data.
